Yasin Patel appointed president and CEO of MHN ACO

As the president and CEO, Patel will work closely with long-time partner Medical Home Network.

Indian-origin Yasin Patel has been appointed president and chief executive officer (CEO) of the MHN Accountable Care Organization (MHN ACO, LLC), a network of 16 Cook County non-profit provider organizations.

As the president and CEO, Patel will work closely with long-time partner Medical Home Network, which founded the MHN ACO in 2014 and recently renewed its management services agreement to provide care enablement services for the ACO.

According to a press statement, Patel holds over a decade of experience in the healthcare industry and value-based care and clinical quality innovation.

Expressing confidence that Patel is a valuable team member, managing director, president and chief executive director (CEO) of Erie Family Health Centers and chairman of the MHN ACO board of managers, Lee Francis said, "Yasin is a leader in the healthcare industry and has a proven track record of working with provider organizations to improve care outcomes and advance innovative health solutions."

Commenting on the new role, Patel said, “I look forward to being part of an organization with a strong mission rooted in culturally competent, whole-person care and a demonstrated track record of success."

Karen Janousek, chief population health and growth officer of Sinai Chicago and vice-chair of the MHN ACO board of managers said that Patel's expertise with the Medicaid population and implementing unique strategies to improve health outcomes will make him an invaluable member of the team.

Patel has served in leadership roles at Signify Health, overseeing clinical strategy and quality for an innovative in-home diagnostic program serving Medicare and Medicaid members. He also managed value-based care contracts with hospitals, health systems, and provider groups nationally as part of the Center for Medicare and Medicaid Innovation’s (CMMI) Bundle Payment for Care Improvement Advanced (BPCIA) program.
